Complete Your Credit Report
- For youth 14-17 years old: DHR will complete a Credit Report for you! Talk to your case worker to see if your credit report has been completed. Your case worker should have your credit report. View your credit report and discuss what it means.
- For youth 18-21 years old: Complete your credit report online by visiting www.annualcreditreport.com. You can also fill out the Annual Credit Report Request Form. This is a paper request form to have your credit report sent to you by mail. Please read and follow the instructions on the form. Please work with your case worker on completing your form. If you have any questions, talk to your case worker or your Independent Living Coordinator.
Have information on your credit report that is SUSPECT?Here's what you can do:
- Visit: ID THEFT
- Complete the Identity Theft Affidavit or Uniform Minor Status Declaration. (For youth 14-17)
- Read the Taking Charge - What To Do If Your Identity is Stolen Toolkit to get instructions on how to correct any discrepancies on your credit report.
